Was Roland having a momentary dizziness and thinking about the horn always there in the beginning?

No it wasn't. I recently re-read the whole series however I'd lost my copy of The Gunslinger (which was a fairly new edition) and only had a beaten up old copy that my boyfriend picked up in a second hand bookshop, it was published in 1982 and I was really surprised to find that there is no mention whatsoever about the dizziness or horn at the beginning.
What I'd like to know (if anyone can answer this!?) when did sai King decide how the books would end, or did he always know? In the afterword of this edition he basically says he has no idea what is going to happen but that Roland will eventually reach the dark tower winding his horn. Is this a clue to the story's ending or did he really not knowat that time? Answers on a post card.....

king never knew how it would end, in my copy, there's note at the end of each novel that tells us secrets about writing the dark tower serie.
