Paul's Thumb

So, I recently got a copy of the audiobook version of Misery. Somehow though, I got two copies of one of the parts and would like to know if I'm missing something or if it was just a fluke and sent me that part twice.

The last part I hear is "Kill You. Goddess. Then there was nothing but nothing" and the next part is "3 - Paul."

I'm confused because it talks about him missing a thumb, but I don't remember hearing anything about him getting his thumb cut off. Am I missing a part? Or is that just how it is?
