Did he actually die in the surf accident and the room was like his version of hell or limbo?

Did the clerk put drugs in his drink?

I agree. It's Evil that seems to manifesting in the room 1408 in that hotel...at the moment. Who knows where or in what manner it will show itself next. But, if you wanna go around looking for something that is better left alone, you have no one to blame but yourself.
