Could Someone Clarify the Alternate Ending?

OK I guess I'm slow...and a hopeless romantic...

But the alternate ending SEEMS to me to say that in an alternate string of some kind he DID end up with Sadie...albeit under a different name...as she called him her "guardian angel."

But if that is the case, how did he end up back there, when he actually decided not to go back?
